Job Description

This position coordinates a comprehensive patient care plan under the supervision of the Executive Director. The Medical Case Manager will serve, as clinic liaison in the community, will work with EIS Workers and different agencies to maintain close contact with patients and ensure they remain in HIV care. Duties include harm reduction, insurance assistance, adherence counseling, care coordination, linkage and reengagement, and referral to core and supportive services.


  • Provide medical case management including treatment adherence counseling according to Part B Ryan White Standards of Care.
  • Links and reengages newly identified clients and those who are lost to care.
  • Serves as the principal liaison between patients and medical providers and between Meharry Community Wellness Center and community agencies.
  • Monitors clients who are homeless, incarcerated, institutionalized or hospitalized and coordinates their transition and discharge.
  • Refers clients to drug treatment and rehabilitation facilities and monitors outcomes.
  • Performs other related duties as assigned.

  • Strong interpersonal and communication skills and the ability to work effectively with a wide range of constituencies in a diverse community
  • Clerical, word processing, and office skills.
  • Skill in the use of personal computers and related software applications.
  • Skill in electronic medical record systems, especially in the CAREWare system
  • Good records maintenance skills.
Required Experience
  • Bachelor’s degree with a minimum of two (2) years of directly related experience is required. Master’s degree preferred.
  • Knowledge of Ryan White programs and familiarity with medically underserved minority communities is highly desirable.
